Initial Paperwork 

To get started please download and complete an Initial Paperwork Packet (IPP). Once you download your IPP (to computer or tablet), you can complete this packet using Adobe Acrobat Reader DC (a free program that includes the “Fill & Sign” feature). If you’re not familiar with it, you can expect to quickly learn this PDF overwrite feature. It allows you to hover your cursor over preset text or check boxes to enter your typed answers, place checks in selection boxes, and apply your own signature (if you have a touchscreen). If you have trouble with it, please contact me to arrange an orientation call.

Since any communication between me and any client is protected health information, I want to help you communicate with me in ways that ensure privacy. Thanks for your cooperation in using private means of communication.

To ensure privacy when you send me your completed packet, you can attach it to a message you send me using the “Contact Us” page on this website. Or you can attach it in a reply to a Hushmail email that I’ve sent you ( Or you can fax it to 480.247.5521. You can also print the IPP to complete it then scan and send it to me. If you cannot scan it, you can of course mail it to me. It is important for me to become familiar with your information before we meet. So, the timing of our first meeting depends a little on how you can send me your packet.

Anyone 15 or older who meets with me needs to complete an IPP. That applies to anyone meeting with me one-on-one or in conjoint meetings with a partner or family member. 

If anyone attending sessions wants or needs me to provide/request information about you to/from another person or agency, then use Release of Information.
